Admission Forms for 2018/2019 Academic Session are available for the following courses:

    1.    CHEW (Community Health Extension Worker).

    2.    JCHEW (Junior Community Health Extension Worker).

    3.    National Diploma in Environmental Health Science.

    4.    Pharmacy Technician.
    5. National Diploma in Science Laboratory Technology.

MINIMUN ENTRY REQUIREMENTS:

1.    CHEW – Community Health Extension Worker:

a.     WAEC, SSCE, NECO or GCE O/L with (5) credits at not more than two sittings which must include English language, Mathematics, Biology, Chemistry and Physics

b.    Junior Community Health Extension Worker (JCHEW) with 2 year post qualification experience in addition to the above requirement mentioned, and applicant must be registered with the Community Health Practitioners Registration Board of Nigeria (CHPRBN) and Possess a valid Practice Licenses.

2.    JCHEW – Junior Community Health Extension Worker:

WARC, SSCE, NECO or GCE O/L with (3) credit in English, Mathematics, Biology and Passes in any other 2 subjects in not more than two sittings.

3.    National Diploma in Environmental Health Science:

a.     WAEC, SSCE, NECO or GCE O/L with (5) credits which must include English Language, Mathematics, Chemistry, Biology/Health/Agricultural Science and any of Geography, Physics, Economics.

    4.    Pharmacy Technician:

a.     WAEC, SSCE, NECO or GCE O/L with (5) credits which must include English Language, Mathematics, Biology, Chemistry and Physics in not more than 2 sittings.

   5.    National Diploma in Science Laboratory Technology:

a.     WAEC, SSCE, NECO or GCE O/L with (5) credits which must include English Language, Mathematics, Biology, Chemistry and Physics in not more than 2 sittings.

HOW TO OBTAIN THE ADMISSION FORM:

    1.    Visit Davmaxsun College of Health Technology at No. 40 College Road (Elu-Agu), Obukpa and buy the form at the cost of N6,000 within the school premises.
